A team leader will provide you with a folder which includes:
One bereavement intake sheet
(information form)
Name of deceased, wake, mass, burial info, etc.
One copy of the "Standard
Funeral Mass"
Bible selections for readings, responsorial, music selections
One letter regarding donations
Directions to our church
A copy of the "intake form" will be copied and/ or emailed to you, which will include a contact phone number for our representative. Please feel free to contact us as needed. We will do our utmost to assist and welcome you and your family before, during, and after the ceremony.
